What Is INR and Why Does It Matter?
INR (International Normalised Ratio) measures how long your blood takes to clot compared to average. A normal INR is 1.0. On warfarin, the target is usually 2.0–3.0, meaning your blood takes 2–3 times longer to clot than normal.

How Warfarin Actually Works
Your liver builds four clotting factors — II (prothrombin), VII, IX and X — and it cannot finish any of them without vitamin K. Each one needs a chemical finishing step, gamma-carboxylation, that uses vitamin K up and converts it to an inactive form. Normally the liver recycles that inactive form straight back into usable vitamin K using an enzyme called vitamin K epoxide reductase.
Warfarin blocks that recycling enzyme. Vitamin K is not destroyed, it simply stops being regenerated, so the liver runs short and the factors it releases are unfinished and cannot clot properly. This is the single fact that explains everything else on this page: warfarin does not thin blood that is already in you, and it does not touch clotting factors that are already circulating. It only affects the new ones being made from now on.
Why Your INR Lags Days Behind Your Dose
This is the part patients are most often not told, and it causes real harm when it is missed. Because warfarin only affects newly made factors, the ones already in your bloodstream have to break down on their own before the drug’s full effect shows. Those factors disappear at very different speeds:

The same lag runs in the other direction. When your dose changes, the INR you measure two days later is not yet showing the result of that change. Anticoagulation clinics generally wait about a week before judging a new dose, and re-testing sooner tends to produce a correction on top of a correction — the most common way a stable patient is made unstable.
Why Consistency Beats Avoidance
The instinct on being told vitamin K opposes warfarin is to cut out green vegetables. That is the wrong move, and it makes control worse rather than better. Your dose was worked out against your usual diet. It is the change that shifts your INR, not the vegetables themselves.
A patient eating a large salad most days is straightforward to dose. A patient who eats none for a fortnight and then has three in a week is not, and will swing in both directions — the eliminated vitamin K raises the INR and the sudden return drops it. Reviews of dietary vitamin K in warfarin therapy make this point directly: steady intake supports stable anticoagulation, and the wide differences in dose between individuals are driven mostly by genetics, principally variants in CYP2C9 (which clears warfarin) and VKORC1 (the recycling enzyme warfarin blocks), rather than by diet.1
Why Antibiotics Are the Common Trigger
Antibiotics raise INR by two routes at once. Some of your vitamin K comes from bacteria living in your gut, and a broad-spectrum antibiotic reduces that supply. Several antibiotics separately interfere with the liver enzymes clearing warfarin, so more drug stays active. The two effects compound, which is why a course of antibiotics is one of the most reliable ways for a stable patient to end up over-anticoagulated.
Illness itself contributes. Fever raises the breakdown of clotting factors, and someone unwell is usually eating less, so their vitamin K intake has quietly dropped as well. If you are prescribed antibiotics, say that you take warfarin, and expect an INR check within about a week rather than waiting for your next routine appointment.
What Good Control Looks Like
A single INR reading says less than people assume. What predicts outcomes is the proportion of time your INR spends inside your target range, often called time in therapeutic range. Someone whose readings alternate between 1.4 and 4.2 can have a perfectly normal-looking average while spending most of their time either unprotected or at raised risk of bleeding.
This is worth asking about directly at your review. If most of your readings sit inside range, occasional small deviations are not a crisis. If they swing widely, the underlying cause — missed doses, changing diet, alcohol, an interacting medicine, or an inconsistent brand or timing — matters far more than any single result.
